**By submitting this form, I agree to enroll my child as indicated above. I agree to pay the non-refundable enrollment fee of $100 per child. I understand that my child’s spot in the program is not secured until their enrollment fee has been paid. **A supply fee of $80 per child per year is due in August. **Enrollment packet and forms will be available at open house in August and students must submit the enrollment packet to attend school.
